Chinnari Arogyam referral Confirmation protocol.

As per the instructions of
Commissioner of Health & Family Welfare and Principal Secretary at ‘L’
Block, Secretariat on 7.8.2015, instructed the following points:
1. The children already screened by AWW,
ASHA (community) for 4D’s at Anganwadi centers are to be confirmed by the
Medical Officer at PHC shall be initiated by 12th of August’2015.
2. The out-patient timing for further
confirmation of disease code at PHCs shall be 11.30 am to 4.00 pm.
3. A separate register (SHP Referral
Register) in which the details of confirmed children and to which referral
center referred and cash receipt details with OP number shall be written
clearly in the register by the concerned. Which includes OP number, Child &
Parent name, house survey number, Aadhar number of parent and cash receipt
details.
4. Not exceeding 25 children who have
already been screened for confirmation at PHC shall be examined per day (Gram
Panchayath wise).
5. Every Gram Panchayath secretary -
Anganwadi center should write the date of Chinnari Arogyam Programme at PHCs on
the notice board at Anganwadi centers.
6. The adopted supervisors (CHO, PHN,
MPHEO, MPHS M/F etc) are all incharge for mobilizing the screened children to
PHC from Gram Panchayaths. The name and contact number should be mentioned on
the notice board at Anganwadi center.
7. ASHA/AWW and Village Organization
(Velugu team) should visit house to house who have been already screened for
further confirmation to intimate the day and timings to the parents about
confirmation of disease code at PHC.
8. The Dy District Medical & Health
Officer of concerned CHNC should supervise the planning, implementation,
further confirmation and financial transactions at PHCs level.
9. The PHC Level confirmation of the
screen children should be uploaded every day in the website cfw.ap.gov.in.
10. The RBSK Coordinator, Social worker
and DEIC team to monitor and effective implementation of programme.
11. The DEIC should be prepared for
Medical & Surgical Management and for further referral to these confirmed
cases.
12. The District Medical & Health
Officers should instruct all Programme Officer and Medical team to identify the
gaps and take necessary steps for effective implementation of Chinnari Arogyam.
13. Regarding the DEIC at KGH,
Visakhapatnam, AP will be inaugurated by honorable Chief Minister so proper
arrangements and DEIC completion in child friendly manner.
14. The expenditure towards travel cost
(bus fair only) and mini meals (amount) will be provided to the child and the
parent. Orders may be issued on Monday.
