            Sub:- APM&HS- Sanitation-  Maintenance of Sanitation in OP Room and PHC/CHC /Sub
                        Centre  Premises – Certain Instructions- Issued.
           
Ref:- Instructions dated.31-8-2015 of the Collector and District Magistrate, Krishna,
                       Machilipatnam.

            Instructions have already been issued by this office from time to time to keep the OP Room and all the rooms and PHC/CHC/Sub Centre premises neatly and hygienically. In the reference cited, the Collector and District Magistrate, Krishna, Machilipantam has issued instructions to fix the responsibility among the Medical officer and staff to maintain good sanitation in PHC/CHC/Sub Centre premises.  Due to Unhygienic condition, the Rats and Snakes are in the premises of PHC/CHCs/Sub Centre.

            Hence, the following instructions are issued fixing the responsibility among the Medical Officer and Staff in all PHC/CHC/Sub Centre Level to maintain Good sanitation and to keep the PHC/CHC/Sub Centre premises neatly and hygienically.

The Medical officer is responsible to keep the OP Room and Bath Rooms neatly and Hygienic condition. He/She is also responsible to provide the water facility and Drinking water.

They are responsible to keep the Supervisor Rooms neatly and Hygienic condition.

Pharmacist is responsible to keep the Pharmacist room and Stores neatly and Hygienic condition.

Staff Nurse of PHC/CHC
Operation Theatre, ward and Labor Room
 Staff Nurse is  responsible to keep the OT, Ward and labor room neatly and Hygienic condition

Lab Technician of PHC/CHC
Laboratory Room
 Lab Technician  is  responsible to keep the laboratory  room neatly and Hygienic condition

Medical officer and Supervisors of PHC/CHC
PHC/CHC surroundings and premises 
They are   responsible to keep the entire PHC/CHC surroundings and Premises  neatly and Hygienic condition

Sr.Asst Room and to maintain the record neatly with up to date entries
Sr.Asst   is  responsible to keep the Office  room neatly and Hygienic condition and to maintain the record with up to date entries.

MPHS(F)(M)
Sub Centre Rooms
MPHA(F) and (M) are  responsible to keep the Sub Centre room neatly and Hygienic condition and to maintain the record with up to date entries.


            Once again, all the Medical Officers are instructed to follow the above instructions and to give instructions to the staff working in their institutions. The Medical officers and staff as noted above are responsible to maintain the good and hygienic sanitation in their respective rooms and PHC/CHC/Sub Centre premises. The Special Officers will visit all the PHC/CHC/Sub Centres in their jurisdiction on every Thursday.

            During their visit, they will verify the sanitation condition in PHC/CHC/SC premises and all the rooms of PHC/CHC/SC and they will inform the sanitation condition to the District Collector directly. If any lapses found in sanitation condition in PHCs, CHCs or Sub Centres , the District Collector has issued instructions  to place the responsible person under suspension with immediate effect.

            During the course of video conference on 31-8-2015, the District Collector has issued instructions to clear the jungles and bushes in PHC/CHC/SC premises before Thursday ie. 3-9-2015. If any unused and condemned articles are in PHC/CHC/SC, it should be conduct auction as per rules. As such no bushes, jungles or waste material are found in PHC/CHC/SC. If any found, the drastic disciplinary action will be taken against the responsible person.

            Hence, the Dy.DM&HOs in the district are requested to monitor and see that all the Medical officers are maintained good sanitation in all the PHC/CHC/Sub centers and submit the report to this office.

            If Pharmacist post is vacant or on leave, the Staff Nurse will be the responsible for stores.

            This should be treated as most urgent and Top Priority.
